At Pathways Hospice (Newest Start Up), our associates are our most valuable asset. We are located in Houston, TX (Harris County). Ultimately, we will serve the vast majority of Harris County initially, our service will start in the East and Southeast Region of Houston at initial phase of service. (Baytown, LaPorte, Clear Lake, etc.) We are excited to be able to contribute to this grow in this amazing healthcare direct-community with our newly formed company, contributing to all patients with a continuum of care and long-term care services. RN Hospice Case Manager The Job A hospice RN case manager plays a crucial role in providing compassionate care to patients with terminal illnesses, coordinating their treatment plans, and supporting their families throughout the end-of-life journey. Responsibilities * Patient Assessment; Care Coordination; Direct Nursing Case; Education and Support; Documentation and Compliance; Advocacy Must Haves

• RN Registered Nurse in Texas or Compact state in good standing on license. A valid RN license is mandatory, along with certifications in hospice and palliative care being advantageous.
• Experience with RN Case Management in Hospice of at least one (1) year. Prior experience in nursing, particularly in hospice or palliative care settings is needed.
• Skills in crisis management and interdisciplinary teamwork are essential.
• Typically, a hospice RN case manager holds a Bachelor’s or Master’s Degree in Nursing, with relevant coursework in palliative care and healthcare management. Not required but a plus!
• Must have experience in HCHB emr.

Work Environment * Hospice RN case managers often work in home health settings, providing care in patients' homes, which allows for a more personalized approach to end-of-life care.
